- 5-Year Work History and Verification of Dental Education Graduation
- Verification of a Current State License In Good Standing
- DEA/ CDS License Verification – allows dentist to write prescriptions
- Current Malpractice Insurance Verification
- Government Reporting Inquiry – Medicare fraud, Medicare Opt-Out, etc.
- Regular review to verify all criteria remain in good standing
